This assignment involved the development of a Promoting Interoperability (PI) Playbook focused on improving care coordination and closing the referral loop following pediatric urgent care visits. Using a real-world clinical scenario, the project identified gaps in referral communication that can lead to delays in care and patient harm. The playbook outlines standardized workflows, data exchange requirements, and evidence-based strategies to ensure referrals are electronically transmitted, acknowledged, and completed.

Explanation:
This playbook takes one real workflow failure, a missed orthopedic referral, and redesigns it using FHIR, C-CDA, and closed-loop tracking, so it's centered on interoperability and care transitions.
